From Morris Township:

Morris Township Needs Your Voice!

The Morris Township Health Department is excited to partner with the North Jersey Health Collaborative (NJHC) to provide you with the opportunity to share what health looks like in your neighborhood!

The NJHC is a group of more than 150 organizations working together to create healthy communities and healthy people. Every three years, the NJHC tries to figure out what topics to focus on to improve health across northern New Jersey.

To make sure we’re choosing the topics that really matter, we’re asking YOU to share your opinions and experiences.

The survey is easy to complete and it only takes about 12 minutes! Your answers are completely confidential and it’s up to you whether or not you want to share your voice.

By completing the survey, you can also enter for a chance to win $100 or the grand prize…a football signed by the New York Jets!

You can complete the survey online using this link or you can request a hard copy.

Hard copies may be found in the Morris Township Municipal Building lobby and Health Department.  We can also mail a hard copy to your home.  Please call 973-326-7390 or email here  if you would like to have a hard copy mailed to you.

All hard copies may be returned to the Morris Township Health Department.  The survey is available in English, Spanish, and Arabic.

If you have questions about the survey, you can contact the Morris Township Health Department, Elizabeth Zalme, REHS/Health Educator  or email here

Thank you for your participation and for working with us promote health in your neighborhood and across northern New Jersey!
